Two quick questions

1. when were filing our POA’s & Affidavit of ownership does it have to filed in the county in which you were born or could it be the current county you located now ?

2. I was a tad bit confused on the passport templates I received , so should I not attach my name change paperwork w/ a judges seal with the passport application ? Also should I enter a SS# in order so I wont be denied ?
